Torticollis, also known as wry neck, is the clinical term for a rotated or awkward positioning of the head due to the decreased range of motion and integrity of a critical neck muscle called the Sternocleidomastoid. This muscle is responsible for head rotation and head tilt. 

Torticollis can occur following the birth process or become evident as a baby develops some head control (congenital torticollis). It can also emerge at any age following an injury or develop when extreme strain is placed on the neck musculature (acquired torticollis). 

Torticollis may impact both the gross and fine motor development of a child. Visual tracking, upright head position, and upper extremity movements necessary for feeding and play may also be impacted. Flattening of the baby’s head (plagiocephaly) and facial features may also develop if untreated. 

If your baby is turning their head to one side, you notice a significant head tilt, or there is visual flattening of his or her head, an evaluation for torticollis is a good idea.

How therapy treats torticollis 

Working with a therapist starts with an initial evaluation and interview to understand how it is impacting development. Education about this condition is a key component of treatment in order to guide caregivers in proper positioning while feeding, sleeping, and transporting.  

Therapy may include a home program of stretching exercises to help increase your child’s range of motion and strengthen the neck muscles. These may include passive and active stretches of the neck and shoulder muscles. These stretches are designed to strengthen muscles that are used to maintain good posture. Even for infants who do not seem to be strong enough to reliably hold their own heads, these stretches and exercises can correct the problem quickly. 

Creative ways to incorporate tummy time and routine exercises may be explored in order to treat torticollis yet provide the infant with a playful experience. In addition to these targeted areas, environmental changes may be recommended such as repositioning of toys, diaper tables, and cribs in order to maximize outcomes. Your therapist will also provide you with an additional referral if a cranial helmet is recommended.
